Community Health Survey 2025

Thank you for your interest. This survey will help us learn more about people’s health in our communities. Your answers will help us understand local health needs and improve care for Virginians and West Virginians.

The survey takes about 25 minutes, is completely voluntary, and you can skip any questions you don’t want to answer. Your answers will be kept private.
