2025 Community Health Survey

Thank you for your interest. This survey will help us learn more about people’s health in our communities. Your answers will help us understand local health needs and improve care for Virginians and West Virginians.

If you received a letter or postcard in the mail, your home was randomly chosen to take the survey. To make sure we hear from many different people, please ask the adult in your home with the most recent birthday to fill out the survey. It takes about 25 minutes, is completely voluntary, and you can skip any questions you don’t want to answer. Your answers will be kept private.

Click the "Take Survey" button at the top of the page or below to take the survey. You will be prompted to enter your unique access code. This is the code listed on the letter or postcard you received.
