UIP INTERN PERFORMANCE EVALUATION

Purpose
As a UIP sponsor, you have an opportunity to review and comment on your intern�s performance twice each semester in order to:
  • Assess an intern's efficiency and effectiveness in achieving performance objectives;
  • Assist in the promotion and development of professional competence of your intern;
  • Help your intern to identify and understand his or her strengths and weaknesses;
  • Provide your intern with direction for continued professional growth and development; and
  • Contribute to an intern's final grade.

Evaluation Guidelines
  • Performance evaluations are conducted openly and with the intern's full knowledge;
  • Evaluations are completed twice a semester: mid-term and final;
  • Evaluations are confidential information and must be treated as such; Evaluations must be based on your personal observations of your intern. That is, the evaluator must be a person in a position to observe the intern's work processes and outcomes.
NOTE: If you have received an evaluation notification and someone else is directly supervising this student, e-mail Karen A. Farber at kfarber@virginia.edu with the name and e-mail address of the person who is supervising the work of this intern.
  • In addition to rating the intern�s performance via the various items, in the space labeled � PLUS (+)/DELTA (∆ ), please provide written comment using
  • PLUS (+) to indicate what is good and correct about your intern's performance � things you like and want encourage the intern to continue;
  • DELTA (∆ ) what you would like to see changed and/or improved in your intern's performance.
  • Finally, please schedule a time to discuss results of your evaluation with your intern.
  • Schedule a conference with your intern no later than 48 hours after you submit the evaluation; and
  • Be sure to provide the intern with a hardcopy of the evaluation at the time of the conference

Instructions


  • Click on the pull-down box to the right of "name (Last, First) & ID#." A list of all UIP interns will appear.
  • Scroll down until you find the name of your intern.
  • Click on the name to select it.
If the intern's name does not appear on the pull-down list, print this form,complete the evaluation by hand and fax it to UIP at (434) 982-5524.
  • Click on the appropriate button to select the Evaluation Period: Mid-semester or Final.
  • Click on the appropriate button to select the appropriate Semester.
  • Enter your name: Last Name, First Name.
  • Enter your telephone number including area code.
  • Enter your e-mail address.
  • Enter in the date on which the performance evaluation was/will be reviewed with the intern.
  • For each performance criterion click on the pull-down box marked "Performance" and rate the intern's performance as:
  • Not applicable
  • Extraordinary
  • Excellent
  • Very Good
  • Adequate
  • Marginal
  • Unacceptable


    NOTE:
    The "Extraordinary" rating should be used to describe performance that consistently and significantly exceeds all expectations all of the time. Ratings at the "Extraordinary" level are rare and serve as the model of excellence for other interns, in terms of exceptional proficiency, as well as, the exceptional manner in which internship responsibilities are performed.


    The "Unacceptable" rating should be used to describe performance that consistently and significantly falls short of expectations all of the time. Ratings at the "Unacceptable" level are rare and are an indication that substantial correctiveaction is necessary to bring performance to the level of "marginal."

  • When finished, click the "Submit Evaluation" button at the end of the evaluation form to send it electronically to the UIP office. A copy of the evaluation will appear and you can print it to review with your intern.
